Membership Plans with a Primary Doctor to Fit Your Budget and Healthcare Needs

When you sign up for an individual or family plan, you gain 24/7 access to your primary doctor, fixed medical costs, and greater continuity of care. This leads to more-informed healthcare decisions, better lifestyle management, and disease prevention. (Most patients find that our cost for an entire year of care is less than the cost of one annual physical at traditional practices.)

You choose the plan that’s right for you, and we’ll assure full transparency of billing and timely communication so there are no surprises. You will know your medical fees and what’s covered ahead of time.

Memberships are paid monthly. Members must also submit a one-time enrollment fee of $100.00 per household.
